Gynecomastia surgery

I want to have a flatter lower chest which does not cause bumps when wearing a t shirt.

I am very interested in surgery and i think its exactly what i need. I have a couple of questions,

Does the full cost of surgery have to be paid in full or can you pay in monthly installments? As it would suit me better to pay over a period of time

Once you have had the operations,would you have to re visit the surgey at a later date?
